How can I submit our Group Proof of Disability form remotely?
Private schools and other organizations that are not U.S. public schools must add Members and Sponsors to their Bookshare organizational accounts using the Group Proof of Disability form.
We understand that at this time, printing, signing, scanning, and emailing this form may be difficult or impossible. Therefore, we are allowing a temporary workaround.
We are allowing the Competent Authority to “sign” the completed Group Proof of Disability form by sending it to us by email along with the wording and information we have outlined below.
- Download a copy of the Group Proof of Disability form from within your Bookshare organizational account. After you log in, go to “My Bookshare” and then select “Members,” and you will see a link to the blank form that you can download.
- Either you or the Competent Authority should digitally complete all of the required fields on your Group Proof of Disability form and then save the electronic file.
- IMPORTANT – Please ask the competent authority to email that completed digital form to GroupPOD@bookshare.org, and include the following in their email:

I attest, under penalty of perjury, to the physical basis of the visual, perceptual or other physical disability limiting these applicants’ ability to effectively use standard print, and that I have the professional qualifications to make such a certification and/or have legal access through my organization to existing written documentation attesting to this fact.

Please be sure that the completed Group Proof of Disability form is included as an attachment to the email, even though it will not be signed. This provides the necessary proof to our auditors that the person sending the email was aware of what they were signing.
Once we receive the email with these items, we will add the members to your account as quickly as possible so that you can begin providing Bookshare books to them.
