How do I provide proof of disability for Individual Memberships?
There are several methods for providing Proof of Disability when completing an Individual Membership for Bookshare.
Students of schools with Organizational Memberships:
If your school has already submitted your Proof of Disability to Bookshare as part of its account, your teacher or sponsor at the school can log in and print a special registration form specifically for you that will register you for Individual Membership without the need for additional proof of disability.
National Library Service (NLS) patrons:
Select My Account, select the Proof of Disability link, and complete the online Disability Verification Request form.

Learning Ally (Formerly RFB&D) members:
Select My Account, select the Proof of Disability link, and complete the online RFB&D Disability Verification form.

All other members:
You will be prompted during the signup process to download a personalized Individual Proof of Disability form which must be completed by a competent authority and submitted to the Bookshare membership team. (Note, we are temporarily offering a paperless option for submitting the POD form.)
You can also download a personalized Proof of Disability form from your account if required. Simply:
- Log in to Bookshare
- Select the My Account link
- Select the Proof of Disability link from the left side of the page
