How do I provide proof of disability?

There are several methods for providing Proof of Disability when completing an Individual Membership for Bookshare.   

Students of schools with Organizational Memberships:

If your school has already submitted your Proof of Disability to Bookshare as part of its account, your teacher or sponsor at the school can log in and print a special registration form specifically for you that will register you for Individual Membership without the need for additional proof of disability.

National Library Service (NLS) patrons:

Select My Account, select the Proof of Disability link, and complete the online Disability Verification Request form.

*Please note, it may take up to 5 – 7 business days for us to complete the verification.

Learning Ally (Formerly RFB&D) members:

Select My Account, select the Proof of Disability link, and complete the online RFB&D Disability Verification form.

*Please note, it may take up to 5 - 7 business days for us to complete the verification.

Canadian members:

Patrons with a current CNIB Identification Card can email, mail or fax a copy of their card to Bookshare.

All other members:

You will be prompted during the sign up process to download a personalized Individual Proof of Disability form which must be completed by a competent authority and submitted to the Bookshare membership team.  

You can also download a personalized Proof of Disability form from your account if required.  Simply:

  1. Log into Bookshare
  2. Select the My Account link
  3. Select the Proof of Disability link from the left side of the page

On this page you will find our options for submitting Proof of Disability including downloading a personalized Individual Proof of Disability form.  If this link does not appear for you please verify on your My Account page that your account's Proof of Disability has been received.
