Renal Pharmacotherapy: Dosage Adjustment of Medications Eliminated by the Kidneys
by Tyree H. Kiser Isaac Teitelbaum Nancy M. Stolpman Gerard R. Barber Larry K. Golightly Bonita A. SimendingerTo promote effectiveness and minimize possible toxicity, the dosage of certain medications must be adjusted in persons with compromised kidney function. Failure to enjoin appropriate dosage adjustments in patients with abnormal or rapidly changing kidney function continues to lead to reports of drug toxicity involving a broad array of renally eliminated medications. This updated edition captures nearly 200 new drugs that have been approved by the FDA since the initial publication of Renal Pharmacotherapy. It also covers new evidence that has emerged regarding the need to adjust dosage of certain older medications that are eliminated by the kidneys. Additionally, it presents new data that are being continuously derived in the areas of patient-specific dose individualization for drugs of all types.Comprehensive, convenient, and evidence-based, this reference closes several identified knowledge gaps and will continue to be the leading collection of dosage recommendations for patients with compromised kidney function.
Renal Physiology and Hydrosaline Metabolism
by Pedro A. Gallardo Carlos P. VioThis volume discusses renal function and the mechanisms by which the kidney regulates the composition and volume of the extracellular fluid. It also highlights the role of the kidney in the development and progression of arterial hypertension. Most textbooks of renal physiology are based in mammalians physiology and mostly human physiology of the kidney, but the authors considered that this book should also include other species to include the broad spectrum of students and researchers in the life and biomedical sciences. In this sense, we included chapters such as comparative osmoregulation in non-mammalian vertebrates and we emphasize that in vertebrates like fish, reptiles, amphibians and birds, the kidneys and extrarenal organs are vital to maintain fluid homeostasis. The purpose of the book is to provide a concise frame of knowledge in a clear and direct language, of the renal function to medical and biological sciences students. In the context of normal renal function, we provide pathophysiological basis for chronic renal diseases and hypertension with the participation of renal vasoactive hormones. This book is used as textbook in several physiology courses for medical, nursing and biological sciences students at the Pontifical Catholic University of Chile, Finis Terrae University, Universidad San Sebastian as well as other universities.
Renal Tubular Acidosis in Children: New Insights in Diagnosis and Treatment
by Ricardo MuñozThis book is a compilation of scientific knowledge regarding the clinical entity known as “Renal Tubular Acidosis in Children” (RTA). It contains the information needed to comprehend the intricate mechanisms of acid-base metabolism, electrolyte imbalance, and complications related to renal tubular acidosis in the pediatric age group, since the acid-base physiology and the clinical disorders associated with it in children are quite different from the adult counterpart. The chapters, written by authors, experts in the field, describe systemic and renal physiology, electrolyte alterations, as well as the pathogenesis and chromosomal mutations leading to the development of RTA. The reader will find the latest information on diagnostic tools and learn how to approach diagnosis, avoiding improper treatment, rickets, nephrocalcinosis, and renal failure. In addition, new insights on classification, the clinical presentation of each type of RTA, molecular biology, and transmembrane protein transporters are covered. This work is primarily directed at pediatric nephrologists and pediatricians. Nevertheless, general practitioners, medical students, residents and fellows at different medical training programs, and other health care professionals of nutrition, growth and development, gastroenterology and endocrinology may surely benefit from it.
Renal Tumors of Childhood

Renin Angiotensin System and Cardiovascular Disease
by Edward D. Frohlich Walmor C. DemelloThe renin angiotensin system plays an essential role on cardiac and vascular functions and their connection in the generation of cardiovascular diseases. Renin Angiotensin System and Cardiovascular Disease covers indispensable aspects such as intracellular signaling, regulation of cell volume in the failing heart, the presence of an intracrine renin angiotensin system, and the influence of prorenin/renin, angiotensin II, angiotensin (1-7) and aldosterone. Furthermore, the role of the mineralocorticoid receptor as an imperative component of the intracrine renin angiotensin system and as a regulator of extracellular action of angiotensin II is described, reinforcing the view that aldosterone inhibitors are helpful in the treatment of heart failure and hypertension. Renin Angiotensin System and Cardiovascular Disease provides readers with an outstanding review of the major topics involving the renin angiotensin system and cardiovascular disease. With contributions by an international panel of experts, Renin Angiotensin System and Cardiovascular Disease presents the most relevant aspects of the role of the renin angiotensin aldosterone system on cardiovascular diseases including hypertension, congestive heart failure, atherosclerosis and diabetes. The volume Editors, Dr. Walmor C. DeMello and Dr. Edward Frohlich, both established specialists in their respective fields, team up to discuss a range of topics from Renin Angiotensin Aldosterone System and Pathobiology of Hypertension to Inhibiting the Renin Angiotensin Aldosterone System in Patients with Heart Failure and Myocardial Infarction.
Reoperative Hand Surgery
by Scott F.M. DuncanReoperative Hand Surgery is a major contribution to the literature, offering a wide range of reoperative options for challenging problems that face the reconstructive hand surgeon after failed primary surgery. Concise and discussing all of the more common issues that hand surgeons often face in performing revision surgery, this invaluable title is unique in that it looks specifically at the problem of what to do when the original index procedure did not go as planned. Topics covered include the hand, wrist, forearm and elbow, and several sections of the book discuss some of the psychological and social factors that also go into the decision process to reoperate or not. The chapters are tightly focused so that the important aspects to consider for reoperation can be quickly reviewed. There is also a component that discusses how to avoid other pitfalls and to minimize as well as manage any complications that may occur with reoperative procedures. Importantly, the book is replete with original art work to help describe and demonstrate how to perform many reoperative procedures. The artwork is simple and straightforward but yet detailed enough to convey the important steps and aspects for the surgical intervention it is outlining. Developed by renowned experts in the hand surgery field, Reoperative Hand Surgery is an indispensable, state-of-the-art reference, offering the busy hand and upper extremity surgeon easily accessible information relating to the evaluation, diagnosis, and possible surgical interventions for the diagnoses discussed.

Reoperative Pelvic Surgery
by William A. Peters Richard P. Billingham Kathleen C. KobashiThe close anatomical proximity of the colon, rectum and anus to the gynecologic and urologic organs often results in a disease affecting one of these organ systems to create complications in another. Many colorectal, gynecologic, and urologic diseases, and the management of their complications, necessitate technically-challenging reoperations. Reoperation on this anatomic area is often necessitated by cancer recurrence, adhesive disease, recurrence of pelvic prolapse disorders as well as in the execution of multi-staged procedures. Divided into three main sections, Reoperative Pelvic Surgery addresses benign and malignant colorectal, gynecologic, and urologic diseases specifically as they are managed in the reoperative setting with a multi-disciplinary emphasis on decision-making, operative strategies, and complications management. Authors discuss important anatomical landmarks, including surgically altered anatomy and function, as well as strategies for the often adhesive nature of the previously operated pelvis. Topics include cancer of the colon, rectum, anus, ovary, uterus, bladder, and prostate, ulcerative colitis, Crohn's disease, rectal prolapse, bowel obstruction, endometriosis, vaginal prolapse, fistulae, incontinence, cystocele, and ureteral strictures. With 200 illustrations and 75 tables, Reoperative Pelvic Surgery will provide a comprehensive resource of technical material not found in standard surgery references. For the colorectal surgeon, general surgeon, gynecologic or urologic surgeon facing the challenges of reoperative surgery in the pelvis, Reoperative Pelvic Surgery will provide the surgeon with the information needed for optimal surgical management of these difficult conditions.
Reparable Harm
by Lois M Davis Dana Scultz M. Rebecca KilburnThe Los Angeles area has the most severe traffic congestion in the United States. Trends in many of the underlying causal factors suggest that congestion will continue to worsen in the coming years, absent significant policy intervention. Excessive traffic congestion detracts from quality of life, is economically wasteful and environmentally damaging, and exacerbates social-justice concerns. Finding efficient and equitable strategies for mitigating congestion will therefore serve many social goals. The authors recommend strategies for reducing congestion in Los Angeles County that could be implemented and produce significant improvements within about five years. To manage peak-hour auto travel, raise transportation revenue, improve alternative transportation options, and use existing capacity more efficiently, they recommend 10 primary strategies: improve signal control and timing; restrict curb parking on busy thoroughfares; implement paired one-way streets; promote ride-sharing, telecommuting, and flexible work schedules; develop a high-occupancy toll-lane network; vary curb-parking rates with demand, enforce the current parking cash-out law; promote deep-discount transit passes; expand bus rapid transit and bus-only lanes; and implement a regionally connected bicycle network. In addition, three recommendations may help, depending on the outcome of current events: evaluate arterial incident management, consider cordon congestion tolls, and levy local fuel taxes to raise transit revenue. Given that some of the recommendations may prove controversial, the authors also outline complementary strategies for building political consensus.

Repeated Measures Design with Generalized Linear Mixed Models for Randomized Controlled Trials (Chapman & Hall/CRC Biostatistics Series)
by Toshiro TangoRepeated Measures Design with Generalized Linear Mixed Models for Randomized Controlled Trials is the first book focused on the application of generalized linear mixed models and its related models in the statistical design and analysis of repeated measures from randomized controlled trials. The author introduces a new repeated measures design called S:T design combined with mixed models as a practical and useful framework of parallel group RCT design because of easy handling of missing data and sample size reduction. The book emphasizes practical, rather than theoretical, aspects of statistical analyses and the interpretation of results. It includes chapters in which the author describes some old-fashioned analysis designs that have been in the literature and compares the results with those obtained from the corresponding mixed models. The book will be of interest to biostatisticians, researchers, and graduate students in the medical and health sciences who are involved in clinical trials.
