Tendon and Ligament Injuries of the Foot and Ankle: An Evidence-Based Approach

by Jarrett D. Cain MaCalus V. Hogan

Injuries of the foot and ankle can be debilitating and lead to chronic instabilities that can impede the daily activities of patients. As a result, it requires a solid understanding of foot and ankle anatomy and pathology in order to diagnosis these types of injuries and establish a clear treatment regimen for the patient to be functional and prevent long-term complications. For the foot and ankle specialist, the understanding of the injury patterns and treatment algorithms continues to evolve with the addition of new information on non-surgical and surgical techniques that are continuing to be introduced to the medical community. However, it is important to determine how the latest developments and treatment advances are disseminated through an evidence-based approach, to allow for proper evaluation of their usefulness as well as how to properly initiate and perform these treatments during patient care. To that end, this book provides a comprehensive overview of the diagnosis and management of muscle, tendon and ligament injuries of the foot and ankle. Opening with a review of diagnostic and imaging procedures, focused, concise chapters then describe the current evaluation and management strategies for a wide variety of soft tissue conditions, including turf toe, Lisfranc injuries, acute and chronic later ankle injuries, subtalar joint injuries, peroneal tendon injuries, and Achilles tendon injuries, among others. Each chapter brings together and reviews the latest literature on the topic, offering evidence-based guidelines for foot and ankle specialists, orthopedic surgeons and podiatrists as well as residents, fellows and all staff involved in the treatment of these injuries and conditions.

TennCare, One State's Experiment with Medicaid Expansion

by Christina Bennett

A history of the struggle among competing stakeholders in one of the oldest and most controversial experiments in US health care policy, a precursor to ObamacareIn 1993, Tennessee launched a reform initiative designed to simultaneously expand the proportion of residents with health insurance and curtail cost increases. It was guided by principles that nearly match those that guided the creation of the Affordable Care Act, also known as Obamacare. Like the ACA, TennCare used corporations, rather than a single government payer, to implement the plan, and it relied on a mix of managed care, market competition, and government regulation. While many states cut back on their Medicaid enrollments from 1993 to 2001, TennCare grew from 750,000 to 1.47 million enrollees. The state was less successful in controlling costs, however. Each major stakeholder group (the state, the managed care organizations, the providers, and the enrollees and their advocates) pushed back against parts of the state's strategy that adversely affected their interests, and they eventually dismantled the mechanisms of cost constraint. The author lays out the four stakeholder perspectives for each period in the history of TennCare and provides a link to difficult-to-access primary documents.

Tensegrity Systems: Basic Concepts, Mechanical Metamaterials, Biotensegrity (CISM International Centre for Mechanical Sciences #2)

by Fernando Fraternali Julian J. Rimoli

This book illustrates the unique mechanical behaviors of tensegrity systems and their applications in mechanical metamaterials, space structures, and biomechanical models. It demonstrates that by controlling the mechanical response of tensegrity structures through internal and external prestress, it is possible to adjust the speed of mechanical waves within these systems, creating tunable bandgap structures. Furthermore, the geometrically nonlinear response exhibited by several tensegrity systems allows for the support of either compression or rarefaction solitary wave dynamics. These behaviors can be effectively utilized to design novel devices capable of focusing mechanical waves in narrow regions of space, as well as innovative impact protection systems. After an introduction to the basic concepts and calculation methods for tensegrity systems and their minimal-mass design, the chapters explore the metamaterial behaviors of tensegrity systems associated with bandgap and solitary wave dynamics; present a mechanical model of flexible tensegrities, illustrating how harnessing the buckling of bars in such systems can result in structures with exceptional energy absorption capabilities, suitable for applications such as planetary landers or lattice metamaterials; and discuss the extreme mechanical behaviors achievable in tensegrity-inspired lattice structures exhibiting both soft and stiff deformation modes. The last chapters address the multifaceted field of biotensegrity, and provide an overview of current rapid prototyping techniques for tensegrity systems, along with a discussion of open questions and research opportunities in the field.

Tensions and Barriers in Improving Maternity Care: The Story of a Birth Centre (Radcliffe Ser.)

by Ruth Deery Mavis Kirkham Deborah Hughes

We have written this book because the story it tells warrants a wide audience. We see the purpose of this book as informing discussion and decision-making around reconfigurations of maternity care, so that planning, communication, management and recruitment can be improved and shared vision articulated and understood.A" Throughout the world, women-centred care is gaining prominence in providing maternity care. Many birth centres open each year to meet this need - but at the same time, many close or are shelved. So why should the turnover in organisations that deliver such a vital service to women be so high, thwarting many midwives from practising as they would wish? This carefully researched and passionate book tells the story of a birth centre that did fail, and the painful but valuable lessons it presents for others. Many of the issues and behaviours illustrated - lack of leadership, support, vision and plain-dealing, and tensions between bureaucracy and women-centred care - will find resonance in maternity services and midwifery experiences in the UK and throughout the world. Tensions and Barriers in Improving Maternity Care is a vital and challenging resource for all midwives, managers and policy makers and shapers with an interest in maternity and women-centred care. "A remarkably detailed analysis of the politics of a birth centre trapped in a medicalised system that threatened and rapidly destroyed it. It is a vivid example of how autonomous midwifery is undermined by an organisational structure in which management focuses exclusively on one model of care." - From the Foreword by Sheila Kitzinger 'I would recommend this powerful book to all supervisors of midwives as it provides profound insights into the impact of loss and grief upon the midwives who are often left feeling isolated and vulnerable when dealing with difficult circumstances.' - Nessa McHugh, lecturer in midwifery at Edinburgh Napier University, and leader of the Preparation and Practice of Supervisors of Midwives programme.

Tenía que sobrevivir (I Had to Survive Spanish Edition)

by Dr Roberto Canessa Pablo Vierci

Now available in Spanish. On October 12, 1972, a Uruguayan Air Force plane carrying members of the "Old Christians" rugby team--and many of their friends and family members--crashed into the Andes Mountains. I Had to Survive offers a gripping and heartrending recollection of the harrowing brink-of-death experience that propelled survivor Roberto Canessa to become one of the world's leading pediatric cardiologists. As he tended to his wounded teammates amidst the devastating carnage of the wreck, rugby player Roberto Canessa, a second-year medical student at the time, realized that no one on earth was luckier: he was alive--and for that, he should be eternally grateful. As the starving group struggled beyond the limits of what seemed possible, Canessa played a key role in safeguarding his fellow survivors, eventually trekking with a companion across the hostile mountain range for help. This fine line between life and death became the catalyst for the rest of his life. This uplifting tale of hope and determination, solidarity and ingenuity gives vivid insight into a world famous story. Canessa also draws a unique and fascinating parallel between his work as a doctor performing arduous heart surgeries on infants and unborn babies and the difficult life-changing decisions he was forced to make in the Andes. With grace and humanity, Canessa prompts us to ask ourselves: what do you do when all the odds are stacked against you?

Terminology, Ontology and their Implementations (Health Informatics)

by Peter L. Elkin

This revised new edition containing numerous new and heavily updated chapters provides readers with the essential information needed to understand the central topics of terminology in healthcare, the understanding of which is an asset to be leveraged in care and research. Twenty-five years ago the notion that terminology should be concept-based was all but unknown in healthcare; now almost all important terminologies are at least partly concept-based. With no general model of what a terminology was or should be, there were no tools to support terminology development and maintenance. Steady progress since then has improved both terminology content and the technology and processes used to sustain that content. This new edition uses real world examples from the health sector to delineate the principal issues and solutions for the field of data representation. It includes a history of terminologies and in particular their use in healthcare, including inter-enterprise clinical and research data aggregation. Terminology, Ontology and their Implementations covers the basis, authoring and use of ontologies and reference terminologies including the formalisms needed to use them safely. The editor and his team of carefully chosen contributors exhaustively reviews the field of concept-based indexing and provides readers with an understanding of natural language processing and its application to health terminologies. The book discusses terminology services and the architecture for terminological servers and consequently serves as the basis for study for all students of health informatics.
