Learning from Paediatric Patient Journeys: What Children and Their Families Can Tell Us (Pediatric Diagnosis and Management)

by Chloe Macaulay Polly Powell Caroline Fertleman

Good healthcare is about more than making a diagnosis and giving the correct medical treatment - the experience of those going through the system is hugely important. As a result, the use of patient stories or “narratives” in learning is gathering significant support and interest. This is a collection of such narratives from children and from the parents of children with a range of long-term or complex conditions. They would be useful for medical students and instructors, but also for anyone working for children with chronic and complex conditions, including nurses and other allied health professionals, as well as psychology students. There are also essays on points arising from a parent and from those involved as a tutor and as a student. A free sample chapter is available via the Downloads / Updates tab on our website.

Lethal Decisions: The Unnecessary Deaths of Women and Children from HIV/AIDS

by Arthur J. Ammann

This first-person account by one of the pioneers of HIV/AIDS research chronicles the interaction among the pediatric HIV/AIDS community, regulatory bodies, governments, and activists over more than three decades. After the discovery of AIDS in a handful of infants in 1981, the next fifteen years showed remarkable scientific progress in prevention and treatment, although blood banks, drug companies, and bureaucrats were often slow to act. 1996 was a watershed year when scientific and clinical HIV experts called for treating all HIV-infected individuals with potent triple combinations of antiretroviral drugs that had been proven effective. Aggressive implementation of prevention and treatment in the United States led to marked declines in the number of HIV-related deaths, fewer new infections and hospital visits, and fewer than one hundred infants born infected each year.Inexplicably, the World Health Organization recommended withholding treatment for the majority of HIV-infected individuals in poor countries, and clinical researchers embarked on studies to evaluate inferior treatment approaches even while the pandemic continued to claim the lives of millions of women and children. Why did it take an additional twenty years for international health organizations to recommend the treatment and prevention measures that had had such a profound impact on the pandemic in wealthy countries? The surprising answers are likely to be debated by medical historians and ethicists. At last, in 2015, came a universal call for treating all HIV-infected individuals with triple-combination antiretroviral drugs. But this can only be accomplished if the mistakes of the past are rectified. The book ends with recommendations on how the pediatric HIV/AIDS epidemic can finally be brought to an end.

Life and Death in Kolofata: An American Doctor in Africa

by Ellen Einterz

When Dr. Ellen Einterz first arrives in the town of Kolofata in Cameroon, the situation is dire: patients are exploited by healthcare workers, unsterilized needles are reused, and only the wealthy can afford care. In Life and Death in Kolofata: An American Doctor in Africa, Einterz tells her remarkable story of delivering healthcare for 24 years in one of the poorest countries in the world, revealing both touching stories of those she is able to help and the terrible suffering of people born in extreme poverty. In one case, a 6-year-old burn victim suffers after an oil tanker tips and catches fire; in another story, Dr. Einterz delivers a child in the front yard of her home. In addition to struggling to cure diseases and injuries and combat malnutrition, Einterz faced another kind of danger: the terrorist organization Boko Haram had successively kidnapped foreigners from Cameroon, and they had set their sights on the American in Kolofata. It would only be a matter of time before they would come for her. Tragic, heartwarming, and at times even humorous, Life and Death in Kolofata illustrates daily life for the people of Cameroon and their doctor, documenting both the incredible human suffering in the world and the difference that can be made by those willing to help.

The Life Cycle of the Corpus Luteum

by Rina Meidan

The book is describes the life cycle of a unique endocrine gland that is absolutely essential for the establishment and maintenance of pregnancy in all mammalian species. The corpus luteum is unique because it is formed and destroyed every (non-fertile) reproductive cycle. When fertilization occurs, the corpus luteum is rescued or maintained. The processes that control corpus luteum formation, demise (luteolysis), or maintenance are just beginning to be unraveled in recent years; they involve diverse cell types and mechanisms. Some of these processes resemble tumor development--angiogenesis, for instance--but interestingly enough, this resemblance is only up to a point. The corpus luteum uses mechanisms that allow its normal, physiological growth and disappearance. Pulling together key research on the corpus luteum, this volume is of interest to both reproductive endocrinologists and comparative physiologists, with clinical relevance spanning comparative animal studies to women's health.

Life on Ice: A History of New Uses for Cold Blood

by Joanna Radin

After the atomic bombing at the end of World War II, anxieties about survival in the nuclear age led scientists to begin stockpiling and freezing hundreds of thousands of blood samples from indigenous communities around the world. These samples were believed to embody potentially invaluable biological information about genetic ancestry, evolution, microbes, and much more. Today, they persist in freezers as part of a global tissue-based infrastructure. In Life on Ice, Joanna Radin examines how and why these frozen blood samples shaped the practice known as biobanking. The Cold War projects Radin tracks were meant to form an enduring total archive of indigenous blood before it was altered by the polluting forces of modernity. Freezing allowed that blood to act as a time-traveling resource. Radin explores the unique cultural and technical circumstances that created and gave momentum to the phenomenon of life on ice and shows how these preserved blood samples served as the building blocks for biomedicine at the dawn of the genomic age. In an era of vigorous ethical, legal, and cultural debates about genetic privacy and identity, Life on Ice reveals the larger picture—how we got here and the promises and problems involved with finding new uses for cold human blood samples.

Life on the Ground Floor

by James Maskalyk

A celebrated humanitarian doctor's unique perspective on sickness, health and what it is to be alive. In this deeply personal book, humanitarian doctor and activist James Maskalyk, author of the highly acclaimed Six Months in Sudan, draws upon his experience treating patients in the world's emergency rooms. From Toronto to Addis Ababa, Cambodia to Bolivia, he discovers that although the cultures, resources and medical challenges of each hospital may differ, they are linked indelibly by the ground floor: the location of their emergency rooms. Here, on the ground floor, is where Dr. Maskalyk witnesses the story of "human aliveness"--our mourning and laughter, tragedies and hopes, the frailty of being and the resilience of the human spirit. And it's here too that he is swept into the story, confronting his fears and doubts and questioning what it is to be a doctor. Masterfully written and artfully structured, Life on the Ground Floor is more than just an emergency doctor's memoir or travelogue--it's a meditation on health, sickness and the wonder of human life.

Light and Specular Microscopy of the Cornea

by Katerina Jirsova

The atlas of the light and specular microscopy of the cornea, particularly of the corneal endothelium presents healthy and pathological corneas as well as corneas prepared for grafting; photographs are taken from donor or patient's corneas. The first section of the atlas shows healthy cornea and its particular layers: the epithelium (superficial and basal cells, subepithelial nerve plexus), stroma and keratocytes, and the endothelium. Blood vessels or palisades of Vogt in limbus are shown as well. The second section that shows corneas processed for grafting is focused on the endothelial layer. The main causes of exclusion of corneas from grafting, such as the presence of dead cells, polymeghatism, pleomorphism, cornea guttata or stromal scars have been shown. The third section shows corneas before and after storage in tissue cultures or hypothermic conditions with the aim to assess suitability of tissue for grafting. Finally, the last section contains photographs of pathological corneal explants.

Linking Health and Education for African American Students' Success

by Nadine M. Finigan-Carr

The linkages between a student’s health and a student’s ability to learn have been well established. Children who are sick stay home; and, children at home cannot learn if they are not in school leading to increased dropout rates among other educational outcomes. However, an understanding of this concept is just the beginning of understanding how education and public health are inextricably linked. In light of this, Linking Health and Education for African American Students’ Success examines health disparities and education inequities simultaneously and moves beyond a basic understanding of health and education in K-12 school programs. The structural inequalities which lead to reduced academic attainment mirror the social determinants of health. Education is one of the most powerful determinants of health, and disparities in educational achievement as a result of structural inequalities closely track disparities in health. These disparities lead to both sub-standard healthcare and reduced academic attainment among children from underserved minorities in the United States, especially African Americans. This book discusses how this may result in children with poorer mental health outcomes; higher school dropout rates; increased risks of arrests and incarceration; higher rates of chronic diseases and mortality; and overall diminished opportunities for success, while providing suggestions as to how to address these issues. This results in an insightful read for researchers, academics and practitioners in the fields of healthcare and education.

Liquid Biopsies in Solid Tumors

by Massimo Cristofanilli

This volume, with chapters written by experts in the field of cancerous tumors, details the key factors associated with liquid biopsies in solid tumors: blood-based diagnostics; circulating tumor cells; enumeration and molecular analysis (association with breast cancer); epithelialmesenchymal transition; detection and monitoring; circulating-free tumor DNA; CTCs and ctDNA; and the exosome. The field of blood-based diagnostics is rapidly evolving demonstrating the possibility of real-time molecular analysis of cancer cells and their phenotype and genotype. Circulating Tumor Cell (CTCs) have demonstrated prognostic and predictive value in advanced cancer and represents a source of tumor cells for transcriptome and genomic analysis. Most recently, the detection of genomic abnormalities in the peripheral blood by sensitive and selective PCR methods (liquid biopsy) opened to the option of a comprehensive blood-based tumor analysis. Similar information can be obtained by analysis of exosome, a natural packaging and messaging system being explored in advanced malignancies. The final frontier is the evaluation of immune cells determinant of innate and adaptive immunity.

The Lived Experience in Mental Health

by Gary Morris

The importance of recognising the knowledge and the needs of service users and engaging them more proactively within the care process is now widely acknowledged, but it is not always clear how this can come about. The Lived Experience of Mental Health highlights individuals’ own lived and felt mental health experience in order to share their expertise about mental health problems and the care offered. This text begins by exploring the importance of engaging with the internal world of those living with various mental health problems and reflecting upon personal narratives as means of expressing and sharing experience, as well as the status of these narratives as 'evidence'. The central section of the book looks at five commonly experienced mental health states: anxiety problems, depression, mood extremes, states of altered reality (linked, for example, with psychosis and schizophrenia) and impaired cognition (linked, for example, with dementia). The chapters look at how the mental state in question is experienced, including the experience of it in the context of the wider world, where health and social care services and the responses of other people play a part. Drawing on personal narratives from a wide range of sources, this text foregrounds the voices of experts by experience and relates them to the academic literature. The narratives collectively convey a breadth of experience including both concepts of struggling and living well with mental health issues. The book ends by outlining resources where a range of first-person narratives can be accessed, from online forums to films, and providing a strategy for teaching and learning associated with the exploration of lived experience narratives. Designed for health professionals working with people experiencing mental health problems, this illuminating text uses personal narratives to emphasise the importance of person-centred care and participation by services users in their own care. It will also be an interesting read for experts by experiences themselves as well as their families and friends.

Living with Disfigurement in Early Medieval Europe

by Patricia Skinner

This book is open access under a CC-BY license. This book examines social and medical responses to the disfigured face in early medieval Europe, arguing that the study of head and facial injuries can offer a new contribution to the history of early medieval medicine and culture, as well as exploring the language of violence and social interactions. Despite the prevalence of warfare and conflict in early medieval society, and a veritable industry of medieval historians studying it, there has in fact been very little attention paid to the subject of head wounds and facial damage in the course of war and/or punitive justice. The impact of acquired disfigurement --for the individual, and for her or his family and community--is barely registered, and only recently has there been any attempt to explore the question of how damaged tissue and bone might be treated medically or surgically. In the wake of new work on disability and the emotions in the medieval period, this study documents how acquired disfigurement is recorded across different geographical and chronological contexts in the period. This book is open access under a CC-BY license.

Living with HHT: Understanding and Managing Your Hereditary Hemorrhagic Telangiectasia (A Johns Hopkins Press Health Book)

by Sara Palmer

Everything you need to know about nosebleeds, arteriovenous malformations, and other symptoms of HHT.Hereditary Hemorrhagic Telangiectasia (HHT) is a rare genetic disorder that causes blood vessel abnormalities in the nose, skin, gastrointestinal tract, lungs, brain, and liver. Nosebleeds are the most common symptom of HHT, but abnormal vessels in other organs, if they are not diagnosed and treated, can lead to serious medical complications, including stroke, hemorrhage, anemia, and brain abscess. Psychologist Sara Palmer, who has HHT herself and is an expert in helping people cope with health conditions, draws on current research as she thoroughly describes the symptoms of HHT, explains how the diagnosis is made (and often missed), and details treatment options. While addressing the medical aspects of HHT, Palmer also reveals how people affected by the disorder can maintain their emotional health, take care of family members, and live life as fully as possible. Enriched with illustrations, personal stories of people living with HHT, a glossary, and contact information for the HHT Centers of Excellence (which provide coordinated medical treatment for people with the disorder), Living with HHT is a complete resource for individuals with HHT and their families. This guide is also essential for health professionals seeking more information about this underdiagnosed disease.

Locomotion and Posture in Older Adults

by Rodrigo Vitório Fabio Augusto Barbieri

This book is an attempt to advance the discussion and improve our understanding about the effects of aging and movement disorders on motor control during walking and postural tasks. Despite these activities are performed daily, there is a high requirement of motor and neural systems in order to perform both tasks efficiently. Both walking and posture require a complex interaction of musculoskeletal and neural systems. However, the mechanisms used to control these tasks, as well as how they are planned and coordinated, are still a question of discussion among health professionals and researchers. In addition, this discussion is more interesting when the effects of aging are included in the context of locomotion and the postural control. The number of older individuals is 841 million in 2015, which is four times higher than the 202 million that lived in 1950. Aging causes many motor, sensorial and neural deficits, which impair locomotion and postural control in the elderly. The severity of this framework is worsened when the aging goes along with a movement disorder, such as Parkinson disease, Chorea, Dystonia, Huntington disease, etc. Therefore, the aim of this book is to highlight the influence of different aspects on planning, controlling and performing locomotion and posture tasks. In attempting to improve current knowledge in this field, invited authors present and discuss how environmental, sensorial, motor, cognitive and individual aspects influence the planning and performance of locomotor and postural activities. The major thrust of the book is to address the mechanisms involved in controlling and planning motor action in neurological healthy individuals, as well as in those who suffer from movement disorders or face the effects of aging, indicating the aspects that impair locomotion and postural control. In addition, new technologies, tools and interventions designed to manage the effects of aging and movement disorders are presented in the book.

The Lomidine Files: The Untold Story of a Medical Disaster in Colonial Africa

by Guillaume Lachenal

Tracing the nightmarish effects of the "wonder drug" Lomidine in preventing sleeping sickness in Africa.Winner of the George Rosen Prize by the American Association for the History of MedicineAfter the Second World War, French colonial health services, armed with a newly discovered drug, made the eradication of sleeping sickness their top priority. A single injection of Lomidine (known as Pentamidine in the United States) promised to protect against infection for six months or longer. Mass campaigns of "preventive lomidinization" were launched with immense enthusiasm across Africa. But the drug proved to be both inefficient and dangerous. Contaminated injections caused bacterial infections that progressed to gangrene, killing dozens of people. Shockingly, the French physicians who administered the shots seemed to know the drug’s risk: while they obtained signed consent before giving Lomidine to French citizens, they administered it to Africans without their consent—sometimes by force. In The Lomidine Files, Guillaume Lachenal traces the medicine’s trajectory from experimental trials during the Second World War, when it was introduced as a miracle cure for sleeping sickness, to its abandonment in the late 1950s, when a series of deadly incidents brought lomidinization campaigns to a grinding halt. He explores colonial doctors’ dangerously hubristic obsession with an Africa freed from disease and describes the terrible reactions caused by the drug, the resulting panic of colonial authorities, and the decades-long cover-up that followed.A fascinating material history that touches on the drug’s manufacture and distribution, as well as the tragedies that followed in its path, The Lomidine Files resurrects a nearly forgotten scandal. Ultimately, it illuminates public health not only as a showcase of colonial humanism and a tool of control but also as an arena of mediocrity, powerlessness, and stupidity.

The Look of a Woman: Facial Feminization Surgery and the Aims of Trans- Medicine

by Eric Plemons

Developed in the United States in the 1980s, facial feminization surgery (FFS) is a set of bone and soft tissue reconstructive surgical procedures intended to feminize the faces of trans- women. While facial surgery was once considered auxiliary to genital surgery, many people now find that these procedures confer distinct benefits according to the different models of sex and gender in which they intervene. Surgeons advertise that FFS not only improves a trans- woman's appearance; it allows her to be recognized as a woman by those who see her. In The Look of a Woman Eric Plemons foregrounds the narratives of FFS patients and their surgeons as they move from consultation and the operating room to post-surgery recovery. He shows how the increasing popularity of FFS represents a shift away from genital-based conceptions of trans- selfhood in ways that mirror the evolving views of what is considered to be good trans- medicine. Outlining how conflicting models of trans- therapeutics play out in practice, Plemons demonstrates how FFS is changing the project of surgical sex reassignment by reconfiguring the kind of sex that surgery aims to change.

The Lotos-Eaters: Aging and Identity in a Yacht Club Community

by Carol A. Warren

As the baby boom generation ages, there are few ethnographies that capture the dynamics of aging. This new book is based on years of participant observation in "the Sands," a beautiful ocean community of well-off individuals and couples seeking the easy life. Yet the community members contend with deep uncertainties about health as they learn to face the realities of death. Identity, sexuality, gender, and conflict play into a sense of "who belongs where," who is counted a friend or stranger in the struggles of old age. Warren shows how the vicissitudes of the aging body center the present and become anchors for the past and future. Expressed in beautiful literary prose, this book moves beyond wealth to explore the realities of aging in poignant new ways that will enliven discussion in courses on Gerontology, Medical Sociology, Inequality, and many others.

Ma Vie en Noir

by Daniel Pedro Cardinali

I wrote this book urged by the overwhelming desire that arises towards the end of life to recapitulate the past. My goal was to summarize my experience of practicing science at the end of the 20th and early 21st centuries in Argentina, a country located far away from the world's leading scientific centers. In the book, I summarize the intricacies of the pineal gland ("the stone of madness") as historical, mystical and medical entity and its entry in contemporary medicine with the description of melatonin. I also reflect on how being associated with an unexplored subject at the beginning of their scientific career impacts the life of a scientist throughout their entire life. Today we know that in humans pineal melatonin begins is released every day late in the evening, and there is evidence that it is the trigger for the sleep process. But the most exciting aspect of melatonin is that it is a substance that is present in all living creatures, from unicellular organisms to plants and higher mammals, a fact that evinces its importance for life. Further, the neuroprotective action of melatonin promises to be crucial for the control of neurodegenerative diseases we face as a pandemic in this century. The discoverer of melatonin, Aaron Lerner, based its name on melano, the Greek word for black, because of its effect on the pigment cells of the skin. As in "La vie en rose", the immortal Edith Piaf song written in 1946, my lifelong work with melatonin could well be called "Ma vie en noir".

Madhouse: Psychiatry and Politics in Cuban History (Envisioning Cuba)

by Jennifer L. Lambe

On the outskirts of Havana lies Mazorra, an asylum known to--and at times feared by--ordinary Cubans for over a century. Since its founding in 1857, the island's first psychiatric hospital has been an object of persistent political attention. Drawing on hospital documents and government records, as well as the popular press, photographs, and oral histories, Jennifer L. Lambe charts the connections between the inner workings of this notorious institution and the highest echelons of Cuban politics. Across the sweep of modern Cuban history, she finds, Mazorra has served as both laboratory and microcosm of the Cuban state: the asylum is an icon of its ignominious colonial and neocolonial past and a crucible of its republican and revolutionary futures. From its birth, Cuban psychiatry was politically inflected, drawing partisan contention while sparking debates over race, religion, gender, and sexuality. Psychiatric notions were even invested with revolutionary significance after 1959, as the new government undertook ambitious schemes for social reeducation. But Mazorra was not the exclusive province of government officials and professionalizing psychiatrists. U.S. occupiers, Soviet visitors, and, above all, ordinary Cubans infused the institution, both literal and metaphorical, with their own fears, dreams, and alternative meanings. Together, their voices comprise the madhouse that, as Lambe argues, haunts the revolutionary trajectory of Cuban history.

Maladies & Medicine: Exploring Health & Healing, 1540–1740

by Jennifer Evans Sara Read

A lively account of medical practices in early modern England: &“Superb . . . an essential piece of social history.&” —Books Monthly It was an era when tooth cavities were thought to be caused by tiny worms and smallpox by an inflammation of the blood, and cures ranged from herbal potions, cooling cordials, blistering the skin, and of course letting blood. Maladies and Medicine tells the story of how the body was understood before the major advances of modern medicine, covering the theory of the four humors and the ways that male and female bodies were conceptualized. It also explains the hierarchy of healers, from university-trained physicians to the women who traveled the country offering cures based on inherited knowledge of homemade remedies, as well as the print explosion of medical health guides, which began to appear in the sixteenth century, from more academic medical textbooks to cheap almanacs. In twenty chapters discussing attitudes toward, and explanations of, some of the most common diseases and medical conditions of the period, the book reveals the ways people understood them and the steps they took to get better. It examines the body from head to toe, from migraines to gout. Case studies and personal anecdotes taken from doctors&’ notes, personal journals, diaries, letters, and even court records show the reactions of individuals to their illnesses and treatments, bringing us into close proximity with people who lived roughly four centuries ago. This richly illustrated study will fascinate those curious about the history of the body and the way our ancestors lived.

Malpractice: A Neurosurgeon Reveals How Our Health Care System Harms Patients and Protects Dangerous Doctors

by John Bechtel Lawrence Schlachter

In 1991, the Institute of Medicine released a landmark report, which revealed that as many as 98,000 patients were dying every year owing to avoidable medical error. More recent research indicates that estimate was, if anything, a drastic understatement of the patient-safety epidemic in the US health care system. In Malpractice, neurosurgeon and attorney Dr. Larry Schlachter makes a case that most patients enter the system without any idea of the risks they face, due to a medical culture that denies there is a patient safety problem. He argues that medical culture actively avoids transparency, perpetuates an atmosphere of blind deference to doctors, and protects dangerous doctors from any accountability.Drawing on 23 years of experience, Dr. Schlachter provides unbelievable stories that illustrate the host of risks patients face whenever they seek diagnostic evaluation or go under the knife. This book provides an all-access pass to the inner sanctums of the health care citadel, exposing the cultural flaws that fuel doctor's egos and outlining the steps every patent should take to protect himself or herself.

The Mammalian Auditory Pathways: Synaptic Organization And Microcircuits (Springer Handbook of Auditory Research #65)

by Douglas L. Oliver Nell B. Cant Richard R. Fay Arthur N. Popper

The auditory system is a complex neural system composed of many types of neurons connected into networks. One feature that sets the auditory system apart from other sensory systems, such as somatosensory or visual systems, is the many stages of neural processing that occur between the ear in the periphery and the cerebral cortex. Each stage is composed of specialized types of neurons connected in specific microcircuits that perform computations on the information about sound. To understand this processing, all the tools of neuroscience must be employed. The proposed text integrates cell biology, synaptic physiology, and electrophysiology to fully develop the topic, presenting an overview of the functional anatomy of the central auditory system. It is organized based on the neuronal connectivity of the central auditory system, which emphasizes the neurons, their synaptic organization, and their formation of functional pathways and microcircuits. The goal of the book is to stimulate research into the cell biology of the central auditory system and the characteristics of the specific neurons and connections that are necessary for normal hearing. Future research on the development of the central auditory including that employing stem cells will require such information in order to engineer appropriate therapeutic approaches.
